Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by finding out the following things about your insurance:
- What is my health insurance coverage and benefits?
- What is my deductible and has it been met?
- Do I have a co-pay? If so, how much is it?
- What is my wellness visit coverage?
We ask that you present your insurance card with every visit.
Cash, check andmost major credit cards are accepted for payment. All co-pays and cash pay visits are due at the time of service.
We ask that if you need to reschedule or cancel an appointment, please let us know 24 to 48 hours in advance. If you do not show up for your scheduled appointment and you have not notified us at least 24 hours in advance, you may be subject to a $45 to $95 cancellation fee.
